Overview
Peak Mental Health (ABN 49 449 460 800) is collecting your personal information so that we can provide you with the services you request, respond to your enquiries and communicate with you in relation to the foregoing.
This Collection Notice explains the types of personal information we collect about you, why we collect it and how it will be used and disclosed.
We have a Privacy Policy that is available on our website or upon request. The Privacy Policy contains information on how you can seek to access or correct any personal information we hold about you, how you may complain about a breach of your privacy and how we will deal with such a complaint. Therapeutic Mental Mental service
As part of providing a therapeutic service to you, your Therapist needs to collect and record personal information from you that is relevant to your situation, such as your name, contact information, medical history and other relevant information. This collection of personal information will be a necessary part of the therapeutic assessment and treatment that is conducted. You do not have to give all personal information, but if you don’t this may mean the therapeutic service may not be able to be provided to you.
Purpose of collecting and holding information
Your personal information is gathered as part of your assessment, diagnosis and treatment, is kept securely and, in the interests of your privacy, used only by your therapist. Your personal information is retained in order to document what happens during sessions, and enables your therapist to provide a relevant and informed therapeutic service to you.
Consent is also required for permission to use your personal mobile number and/or email to
send you appointment reminders/cancellations etc. via the practice management software.
In some cases, your personal information may (strictly on a confidential basis) be disclosed to or accessible by our related companies and third parties that provide us with (or help us provide) products and services, for example appointment reminders/cancellations. In these cases, our providers of IT, data storage, back office and other support services may hold your data securely on our behalf.
We may disclose personal information for other purposes where you provide your express or implied consent for us to do so, or where otherwise required or authorised by law.
Access to client information
At any stage you are entitled to access your personal information kept on file, subject to exceptions in the relevant legislation. The therapist will discuss with you different possible forms of access in your first session.
What if you do not provide your information to us?
If you do not provide us with the relevant and accurate personal or health information we request, it may result in an incomplete assessment that may impact the treatment being provided, and we may not be able to provide you with some or all of our services.
Fees
The cost of a consultation (usually around 50 minutes) is payable at at the time of booking the session via Credit Card Payment through our online payment portal.
If you have a Mental Health Treatment Plan from your Doctor, you may be eligible for a Medicare rebate. Medicare rebates and some Private Health Insurance Fund rebates can be processed at the completion of the session after payment.
Cancellation Policy
If, for some reason you need to cancel or postpone your appointment, please give at least 48 hours notice, otherwise you will be charged half the session fee, to cover associated expenses.
Communication between sessions
Communication via email and telephone in-between scheduled sessions will be kept to a minimum with an expectation that it is limited to administrative issues (i.e. organising appointments, advising of any recent changes, requesting information to be shared with specialists and sharing of resources). Extensive emails will be printed to discuss in the next scheduled session however it is strongly recommended that private information not be shared via this format due to possible privacy and confidentiality issues.
